We ask you to fill out the form below in order to Sponsor a hole for our fund raiser.  The fee to Sponsor a hole is $100.  Other sponsorships are available at various dollar amounts.    This form will be emailed directly to me Joe Herrity.  We ask that all checks be remitted by May 5th or arrange details with Joe Herrity.  Checks should be made payable to: “AOH DIV #3” and mailed to, AOH Division 3, PO Box 416, Doylestown, Pa. 18901.  You can also use venmo  payaohdiv3@gmail.com
